Red- This is your basic information, like your name and your address
Light Blue- This is very important. Without this, we will not be able to make a transaction. The box in the lower bottom is to chose which account you will pay with.
Blue- Optional. If you are going to have roommates, please right the names of the other occupants.
Green- This is for VE students. if you work at a firm, if we find a mistake in this form, we can contact you

After all of this is done, just right your name on the signature line and write the date of the signature. After this, just send an email to:
